Patient Resources

Making your healthcare easier with the Health Portal

It’s easy to manage your appointments, connect with your doctor and view your medical information through our convenient and secure Health Portal – day or night, from anywhere.

Our policies

At HCA Florida Physicians, we aim to provide a comfortable, efficient visit in friendly, caring environment. Our physicians and staff strive to accommodate all patients’ needs as best as possible. To help us help you, please review the policies below.

New patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of X-rays, lab results and your insurance card to your appointment.

Billing

Your co-payment and past-due account balance is due at the time of service. We accept checks, debit and all major credit cards. You can access your records and billing summaries via our patient portal.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate. We recommend that you call your insurance company to verify that we participate with your plan.

Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.

Cancellations or rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

Refill requests

We are happy to help with refill requests during regular office hours, so please call your physician's office to let them know what you need.

Patients rights and responsibilities

We respect our patients' dignity and pride. This section will explain your patient rights and responsibilities. It is part of your patient registration and is an important part of your health care plan.
